Fill out the form below and upload a valid government-issued PWD ID or Senior Citizen ID.

Our team will review your submission and, once approved, we’ll tag your customer account as PWD or Senior.

From then on, whenever you log in to your Pulses Active account, your discount will be applied automatically at checkout — no need to enter a code!
